We want to make sure you’re feeling prepared and ready to go for your first appointment at KC Pain. Below is a list of items you should fill out before coming to your appointment.

  • Patient forms
  • Photo identification
  • Current insurance card
  • List of current medications with directions and dosage
  • List of allergies and reactions
  • Medical history
  • Referring physician’s notes, if applicable
  • Past MRI, CT or x-ray scans, or nerve conduction studies, if applicable
  • Copay or required down payment
  • Worker’s compensation form, if applicable
